BOOK AN APPOINTMENT .

It is important to provide us with all necessary information when you make your appointment.

Information needed when making your appointment:

 

 

  1. First and last name

  2. Home/work address

  3. E-mail

  4. Phone number

  5. Desired appointment date and time

  6. Your request/prescription

  7. Date of birth and RAMQ

OUR POLICIES AND RULES *Make sure to read the policies and rules carefully before booking an appointment! - We only accept requests prescribed in Canada. - Appointments may be moved depending on nurses on the road, traffic, weather conditions, or any situation beyond our control. When this is the case, clients will be notified. - Travel and administration fees are non-refundable. - A $20 deposit is required when confirming your appointment. This is non-refundable if the appointment is cancelled. The deposit for the cancelled appointment cannot be used if another appointment is made at a later date. - No form of violence is tolerated. It is imperative to communicate respectfully at all times, otherwise, we reserve the right to cease all communication with the offending client. - Payment is required immediately after the appointment. We accept payments by credit card, Interac, E-transfer or cash. - It is the client's responsibility to verify that all necessary information is included on the request (doctor's name, signature, license number and fax number or email address to send the results). If any information is missing, it could delay the processing of the tests at the laboratory. The company cannot be held responsible. - Results are available in 24-48 hours. Some tests take longer to be analyzed. When this is the case, the client will be notified. - It is the client's responsibility to verify their request and ensure that all tests are visible when making an appointment. (Front/back) - There is no possibility of reimbursement when the client does not follow the instructions given by the company or the laboratory. - In the event of an analysis rejection by the laboratory, we offer the client to take another sample free of charge. - The service is refundable within 5 days following the treatment only under certain conditions: * If the laboratory does not perform the requested analysis, the analysis costs will be refunded. * If the client does not wish to make another appointment for free in the event of an analysis rejection, he will only be refunded half of the costs of the rejected analysis. * If there is no way to take another sample. - It is the client's responsibility to ensure that he has received all of his results.
